The Toledo Clinic Cancer Centers (TTCCC), a multi-specialty oncology practice and member of ONCare Alliance, today announced a partnership with Canopy, the first Intelligent Care Platform for oncology, to further enhance patient outcomes through continuous care management.
TTCCC will implement Canopy’s comprehensive, EHR-integrated platform to streamline triage processes for more efficient and standardized care, capture reimbursement for this care through a Principal Care Management (PCM) program, and further enhance patient outcomes through continuous symptom monitoring (ePROs).

The Canopy Platform includes ePRO-based remote monitoring, enabling patients to report symptoms between office visits. Care teams can efficiently manage reports and other tasks through a Smart Work Queue that prioritizes issues and enables seamless task management across departments. Oncology-specific triage pathways support nurses in resolving reports, all while auto-documenting key information to the EHR.
Real-world studies of Canopy’s ePRO and remote triage system show high patient engagement—88% at six months [JCO, 2022]—as well as demonstrated clinical outcomes, including an observed 22% lower rate of ER visits/hospitalizations [ASCO, 2022] and up to a 45% observed increase in treatment persistence at 3 months [ASCO, 2022].
TTCCC will also deploy Canopy’s Program Management module to identify and prioritize eligible patients for PCM, streamline enrollment processes and end-to-end care workflows for staff, and automatically capture care time for reimbursement.
